Within the premises of the private entrepreneur MYKITENKO D.O., operating under the trademark "Genetic Center of Maternity and Childhood by Professor Mykitenko," there are specific procedures and patient conduct guidelines. Therefore, we recommend that you familiarize yourself with the Rules for Providing Medical Services, Internal Regulations of the medical institution, price list, qualifications of specialists, certification documents, and other information displayed in the Consumer Corner of MYKITENKO D.O. (hereinafter referred to as PE) near the reception. To save your time, key provisions of these documents are outlined in this Information Leaflet.

1. Medical documentation. Preliminary examination and treatment plan. Consent for medical examination and treatment.

According to the current legislation of Ukraine, the process of providing medical services (assistance) is documented in the relevant medical records. All medical documentation related to your examination and treatment is official and belongs to PE. However, each patient (or their representative with duly executed powers) has the right to review it. All additional information carriers (X-rays, electrocardiograms, test results, conclusions, etc.) used by PE for diagnosis or clarification, even if obtained from other medical institutions, are an integral part of the patient's medical records, permanently stored in PE. Information on the procedure for reviewing, storing, and issuing medical documentation can be found in the Consumer Corner of PE.

Information about your stay at PE (including personal data from questionnaires, medical histories, etc.), the diagnosed condition, conducted medical examinations, etc., is confidential and can only be accessed by you personally or your representatives with duly executed powers, except in cases defined by law.

The Preliminary Examination and Treatment Plan is formulated by the doctor based on the obtained initial data. During the examination and treatment, changes not included in the specified Plan may occur, but they may be necessary to achieve planned results and expedite your recovery. These additional medical services are invoiced separately.

Your consent to receive paid medical services in our PE is documented in relevant medical-legal documents: the medical record, consent for medical examinations, etc.

2. Scheduling visits to the doctor

Visits to the doctor are scheduled in advance according to the patient's agreed examination and treatment plan. The day before each visit, the PE registrar, at the patient's request, reminds them of the scheduled visit by phone. If, for specific reasons, you need to change the date or time of your visit to PE, you must inform the nurse-registrar in advance (preferably no later than 24 hours), as the time is reserved and cannot be used for other patients without prior agreement.

3. Payment terms and amount

Payment for medical examinations, diagnostics, and treatment is made after the doctor's appointment. In certain cases, PE allows patients to pay for services with a delay but no later than the next visit to the doctor.

Diagnostic and control-diagnostic studies are paid according to the current PE price list.

According to the current legislation of Ukraine, you have the following rights and responsibilities related to the process of providing medical services:

1. Right to qualified, effective, and timely medical care (services).
2. Right to choose a doctor, medical institution, and treatment method (examination).
3. Right to provide informed consent for any medical intervention.
4. Right to information about the health condition and diagnosis.
5. Right to information about the treatment method.
6. Right to review medical documentation.
7. Right to confidentiality of medical information.
8. Right to refuse treatment.

Patients' responsibilities:

1. Responsibility to take care of their health and the health of their children, not to harm the health of other citizens.
2. Responsibility, in cases provided by law, to undergo preventive medical examinations.
3. Responsibility to follow the internal regulations of the medical institution.
4. Responsibility to adhere to specific contractual conditions with the medical institution.
5. Responsibility to timely pay for paid medical services.
6. Responsibility to follow the prescriptions and recommendations of the treating physician.
